This song represents to me the death of a loved one, not being able to let go, but not being able to be with them either. And the loss and grief and coping mechanism of talking to the lost loved one. Along with not caring what the neighbours and townspeople are thinking, because he almost believes that yes he is becoming crazy. Talking to some one on the "other side" is a very painful place to be, when no one replies especially if they are "all he had".

Explanation
"
hear me calling?
'Cause every night, I'm talkin' to the moon
Still tryin' to get to you

[Verse 5]
In hopes you're on the other side
Talking to me too
Or am I a fool?
Who sits alone
Talking to the moon

[Outro]
I know you're somewhere out there,
Somewhere far away...
He is asking "do you really hear me, or am I crazy? " and saying "I can't let you go, and I need you." These sentiments are reflected again in the last lines. It's easier to think of a deceased person as some where else rather than totally gone.

I feel that the line "in hopes your on the other side" refers to some one who has died that he is trying to make contact with.
